The North Devon District Hospital Hospital Discharge Team is a group of experienced social workers, social care assessors, nurses, paramedics, and coordinators who work in conjunction with hospital personnel to offer the most suitable discharge options for the patients they encounter.

After discharge from the hospital, the social care assessor assists clients in investigating their options for care and support. This involves evaluating the care and support needs to ascertain the most appropriate discharge pathway. This position necessitates a comprehensive understanding of the Mental Capacity Act, and prior experience conducting mental capacity examinations would be advantageous.

The team is responsible for conducting Care Act assessments for individuals to ascertain their continuous requirements for care and support after they have been discharged from the hospital. You will be instrumental in ensuring that the individuals we assist have the resources they need to succeed in their role as social care assessors.
