Solihull Community Services’ Role:

• Perform distinct clinical pharmacy responsibilities in the pharmacy community.
• Service group. This requires medication revision and management assistance.

• To direct and develop clinical pharmaceutical services for the virtual ward, which provides frailty, long-term patients, and those with respiratory conditions with an alternative treatment approach.

• To deliver a clinical pharmacy service in adherence to standards approved by the trust, which encompass the following:
• Role in community service
• Assess and optimize medications prescribed to patients undergoing intermediate care and services for falls. To detect pharmaceutical errors, perform a medication reconciliation.
• Advise regarding the administration, dosage, adverse drug reactions, precautions, and required monitoring of medications. Provide patients with education and aid in the discharge planning procedure.

• The Contribution of Virtual Wards
• In addition to contributing clinical expertise to virtual wards, function as an essential member of the MDT. Attend meetings of the MDT and ward visits.

• Promoting adherence to prescribing formularies and national, regional, and local clinical recommendations will facilitate rational and cost-effective prescribing.

• Assist in the formulation of policies, procedures, guidelines, and PGDs to guarantee the effective distribution of pharmaceuticals by the Directorate.

Qualifications:

• Degree of Master of Pharmacy (or an equivalent)
• holder of a General Pharmaceutical Council registration.
• Completing the initial year of a postgraduate clinical pharmacy program or an equivalent level of experience.

• Non-medical qualifications for prescribing

Experience:

• Proficiency in delivering presentations or instructing fellow healthcare practitioners or patients.
• Familiarity with national initiatives and suggestions, including NICE.

• Hospital experience providing clinical pharmacy services after registration.
